Nursing home fined again after death of resident

CENTERVILLE, IOWA -- A Centerville nursing home has been fined $15,000 following the death of two patients.

According to reports, inspectors say the Golden Age Nursing and Rehabilitation Center has repeatedly failed to provide the minimum level of care for residents at the facility.

The home was fined $8,000 in September after the death of a 64-year-old woman.

The $15,000 fine is connected to the death, in November, of a 91-year-old woman. An 89-year-old woman also died around the same time.

The owners of the facilty, Stanley and Patricia Birchem of Leon, say they plan to appeal the fine. Patricia Birchem also says issues regarding the two latest deaths have been absolved.
