	KTVO-TV Take-Off and Travel Apple Vacation Riu Playacar Prize #488(all-inclusive) Contest Official Contest Rules The following rules represent the material terms and conditions of the above named contest. By entering, each entrant accepts and agrees to be bound by these contest rules. 1. HOW TO ENTER OR PARTICIPATE a) No Purchase is necessary. b) This contest is void where prohibited or restricted by law. c) THIS IS AN ON-LINE ENTRY CONTEST ONLY. d) To enter, log on to the home page of Heartlandconnection.com for the contest link and fill out the entry form. This is a “+Contest” contest so you get one entry automatically by filling out the form. You can also receive additional entries when you follow us on Facebook and when you share the contest with your friends. e) Log on to the homepage of http://www.heartlandconnection.com and complete the entry form f) No phone calls please. 2. ELIGIBILITY RESTRICTIONS a) This contest is open to residents of the Kirksville/Ottumwa Designated Market Area (DMA) which includes the counties of Putnam, Schuyler, Sullivan, Adair, and Macon in Missouri ; and, Wapello, Jefferson, Davis, and Van Buren in Iowa. In Addition the Nielsen Total Survey counties (NSI) of Appanoose, Monroe, Wayne, Lucas, Marion, Mahaska, Keokuk, Washington, Henry, and Lee in Iowa; and, Scotland, Knox, Shelby, and Linn in Missouri. b) Entrants must be 21 years or older and have a valid Drivers Licensed issued by State of residence c) Employees of Barrington Broadcasting LLC, participating sponsors (Lambert Airport, Kirksville Regional Airport, Cape Air, Apple Vacations), their respective parent companies, affiliates, subsidiaries, suppliers and their advertising and production agencies (collectively, "the Sponsor(s)") and their immediate family members (spouse, parent, child, sibling and their respective spouse) and households of each such employee are not eligible to participate or win a prize. d) All entrants must coordinate with the sponsors to redeem the prizes for booking purposes in order to remain eligible to win. 3. ENTRY DEADLINE DATES a) To be eligible, all online entries must be submitted to and received by Heartlandconnection.com beginning May 2, 2012 at Noon CDT up through Midnight CDT, May 31, 2012. 4. CONTEST PRIZE DISCRIPTION(S) AND ESTIMATED VALUATION(S) Apple Vacations is pleased to confirm (1) three-night trip to Riu Playacar, in Riviera Maya, Mexico for two adults departing from STL, Lambert St. Louis International Airport. Trip includes three-night accommodations, round-trip charter air, transfers to/from resort, non-motorized watersports, all food/drinks and the assistance of an in-resort Apple Representative. Trip is valid for travel through December 10th, 2012. Taxes, insurance and additional expenses (such as optional excursions, spa treatments, phone calls, souvenirs) are the sole responsibility of the winner. a) The prize package includes: i. Number of trips for giveaway: 1 trip for 2 adults ii. Destination: Riviera Maya Mexico iii. Hotel: RIU Playacar (all-inclusive) iv. Number of Nights: 3 v. Departing From: Kirksville and St. Louis vi. Departure Days: Based on charter schedule vii. IMPORTANT NOTICE REGARDING NEW PASSPORT REQUIREMENTS FORINTERNATIONAL TRAVEL– As of January 23, 2007 all international travelers must present a government issued passport for U.S. Customs re-entry after air, land or sea travel. It is the traveler’s responsibility to provide proper documentation. Married or divorced women traveling under names other than what is printed on their travel documents must supply a marriage license and/or divorce decree. viii. • All vacations are approved on a promotional basis & are subject to availability. Blackout dates do apply. ix. • This trip is valid through December 10th, 2012 x. • Travel dates are final and will not be extended. xi. • Travel is not permitted during holiday periods including both 5 days prior to and after xii. • Trips are non-transferable and cannot be exchanged for cash. xiii. • Apple Vacations reserves the right to substitute the vacation with another of equal value, equal Apple rating or within the same hotel chain should any unforeseen circumstance occur. Hotel to be determined by Apple Vacations. xiv. • Trips are valid for 2 adults ONLY per room and do not include any special promotions. NO room upgrades. xv. • Winner must be at least 21 years old or traveling with a legal guardian. xvi. • Employees of participating companies and its properties, sponsors, vendors and their immediate families are not eligible to win. b) KTVO accepts responsibility and is solely responsible for $150 per person tax associated with accepting the prize package as outlined in Section 4.A.X. c) Winner is responsible for any other taxes including a 1099. d) The grand prize is the travel package to RIU CANCUN. However, the sponsor may choose to add other prizes to be awarded as consolation prizes to finalists, at the discretion of THE SPONSORS and are yet to be determined. e) Sponsor(s) make no warranties and disclaim all warranties, guarantees, or representations, express or implied, concerning the condition, quality, or merchantability, or any prize. 5. SELECTION OF WINNER(S) a) On Friday, June 1, 2012, there will be 1 winner selected at random from all valid online entry forms. b) The winners will be notified by KTVO using the contact information provided by the entrant on the entry form and prize pick-up will be arranged. c) If the prize is not claimed within 2 weeks a new winner will be drawn at random. 6. OTHER TERMS AND CONDITIONS a) Entries must be complete and legible in order to qualify. Entries will be void if they are in whole or in part incomplete, illegible, irregular or otherwise questionable at the discretion of the Sponsor(s). b) Multiple uses of a single email address to enter will not be accepted. c) All entries become the property of the Sponsor(s), who reserve the right to use any or all information submitted by entrants for the Sponsor’s marketing or promotional purposes, unless the entrant chooses to opt out. The entries will not be sold by Sponsor(s) to any other party for any other purpose. (make sure entry form offers option to opt out) d) Winner agrees to the use of his/her name and likeness, without any compensation, by sponsor(s) for publicity. By entering this contest, the winner acknowledges that KTVO-TV, and all other sponsor(s) and venues have the right to publicize his/her name, voice and likeness, the fact that he/she won, and all matters incidental thereto. e) Winner(s) accept sole responsibility for claiming the prize and picking it up from the Sponsor(s). Prizes unclaimed within 14 days of award shall be forfeited, and KTVO and the Sponsors shall have no further obligation to the winner(s). If the prize is forfeited, KTVO reserves the right to select an alternative winner of the prize. f) By entering this contest, participants agree that Sponsor(s), their employees, and their agents have no liability whatsoever for any injuries, losses, or damages of any kind which result from use of the prize, or by participation in the contest. Sponsor(s) make no warranties and disclaim all warranties, guarantees, or representations, express or implied, concerning the condition, quality, or merchantability, or any prize. Prize is non-transferable and cannot be exchanged for cash. No substitutions are permitted. g) In the event of a dispute, all decisions made by KTVO-TV are final and binding. h) Contest Rules are subject to modification by KTVO-TV at any time during this promotion. Station/Sponsor(s) reserve the right to change the prize if necessary. i) Sponsor(s) further reserves the right, in its/their sole discretion, to cancel, terminate or modify this promotion if, for any reason, the promotion is not capable of completion as planned, including due to infection by computer virus, technical corruption, force majeure or non-authorized human intervention that compromises or affects the administration, fairness, integrity, security or proper conduct of the promotion. j) Sponsor(s) further reserve the right to continue with the promotion and award the prize(s) even while, in its/their sole discretion, the Sponsor(s) disqualifies any entrant that, in the judgment of the Sponsor(s), has tampered with any entry, caused or contributed to causing any technical corruption in an attempt to compromise the integrity of the promotion. k) Odds of winning are dependant on the number of eligible entries received. l) This promotion is in no way sponsored, endorsed or administered by, or associated with, Facebook.
